Remember, all Vitality Checks must be completed within 60 days of insurance coverage with the Healthy Lifestyles Cooperative/Humana Plan.  This is effective for all employees and covered spouses.  Adult children do not need to complete the HRA.

Pool 3 Health Risk Assessments
Prevea will provide on-site health risk assessments (HRA) coordinated by your employer.  HRA's are a requirement for all employees and any spouses covered under the insurance plan.  Adult children are not required to complete the HRA.

HRA's must be completed by March 31, 2010, OR within 60 days of insurance coverage with the Healthy Lifestyles Cooperative/Humana Plan.
